CHARTTRACE / THE SHORT ANSWER
What is ChartTrace?
ChartTrace is SNF billing and coding software for skilled nursing facilities. It connects the chart to the claim: every proposed code keeps the literal sentence from the resident’s chart that supports it, effective-dated CMS and payer rules decide what is valid, and a person resolves exceptions and approves final release. ChartTrace is in early access.
Who ChartTrace is for
- Skilled nursing facility (SNF) operators and business office managers
- Corporate revenue-cycle (RCM) and reimbursement leaders overseeing several buildings
- MDS coordinators and PDPM / MDS coding reviewers
- Coders and billers who handle exceptions, pre-bill review and claim readiness
What ChartTrace does
- Evidence-backed coding. AI extracts source evidence and proposes coding, each proposal traceable to the exact supporting sentence and its location.
- Effective-dated rules. Deterministic CMS and payer rules determine what is valid, with the rule version and effective date kept as part of the basis.
- MDS, PDPM and HIPPS. Official PDPM/HIPPS grouping for supported fiscal years, on the same evidence trail. See MDS/PDPM coding review.
- Pre-bill claim review. UB-04 / 837I field-level validation, NCCI/MUE and consolidated-billing checks, with anything unsupported held instead of released. See SNF pre-bill review.
- Human control. Routine, evidence-clear work advances; gaps, conflicts and missing support go to a named reviewer, and a person authorizes final claim release.
What ChartTrace is not
ChartTrace is not an AI nurse assistant, a nursing documentation product or a generic claim scrubber, and it is not autonomous AI replacing billing or coding staff. MDS/PDPM review and pre-bill validation are stages it covers within one SNF revenue-cycle platform, from documentation and coding to billing, claims and payment follow-up.
Where ChartTrace is today
ChartTrace is in early access and commercial validation. Direct clearinghouse submission, remittance (835) ingestion, live denial workflows and A/R reconciliation are not yet production-live. Customer ROI and staffing reductions are not yet proven. See the current stage.
